INDIVIDUAL ENTRY FORM

The Championship is open to Amateur Golfers who: -

• Were under the age of 22 at 00.00 hours on 1 st January 2007.
• Were born in one of the Counties comprising the Association (Cornwall, Devon, Dorset, Gloucestershire, Somerset, Wiltshire) or have for the previous period of one year resided in one of these Counties.
• Are members of a Club affiliated to a South West County Golf Union.
• Have a playing handicap of FIVE or less.

The entry will be limited to 60 competitors. Each County may nominate up to eight competitors. Entries to bring the number up to 60 will be accepted from individuals and if there are any in excess of this number, places will be balloted for from those who have not been nominated by Counties.

In the event of a tie, the winner will be decided on the lowest score for the last 18, 9, 6, 3 holes etc.

The Entrance Fee of £10 is to be paid at time of entry.

Players wishing to enter should complete the entry form below and post it, together with a current PLAYING HANDICAP CERTIFICATE, a cheque for £10 payable to SWCGA and a stamped addressed envelope to: -
